Use it on the shotgun you find lying to the left, pick up the shell again when Lee drops it, and finally shoot the zombie's head off. Look at the ground to the right and take the shotgun shell. Back away to escape the zombie attack until you hit the car. Lee will drop them and you'll have to repeat the process to free both wrists. Take the keys and attempt to unlock your handcuffs.
